Party Services Agreement Form
Fees: A $80 non-refundable deposit is required to book your date unless the event is within 7 days. Full payment is due 7 days prior the event. Payments can be made via Zelle (info@anyabraids.com), Credit/Debit card via QuickBooks or in person meet up (in some cases). We charge a 3% fee for Credit/Debit card processing.
Travel Fees: Travel fees apply when the event is more than 30 miles from zip code 91214.
Parking: Client will provide adequate parking and will reimburse for any parking fees. Time spent looking for parking by the stylist/artist will affect the actual schedule length of time. In scenarios where parking is limited, we recommend that the client be prepared to move a car or facilitate some sort of guaranteed parking arrangement.
Set Up
· Client will provide with sun/rain/wind shelter, adequate space, and lighting to work.
· Client is responsible for providing an alternative indoor location in the event of rain or temperatures below 60 degrees or above 92 degrees. Client agrees to pay full amount contracted, even if it is raining.
· Client will provide an access for artists to a restroom and a place to dispose of used paint water and stencils.
Cancellations/Refunds/Rescheduling: Should you need to reschedule, please do so 7 days prior to your event and we will make every effort to meet your needs. If you need to cancel the event, you must cancel within 7 days of the scheduled event, or you will have to pay the TOTAL amount contracted the invoice due to consequential loss of income from your date being held. In the rare and extenuating circumstance that the stylist/artist has to cancel, a comparable replacement hair stylists/artist will be sent in her place, or a trainee will be sent and discount given. Should a replacement not be available, client’s deposit will be refunded.
Extreme Adverse Working Conditions: Our stylists/artists have the right to cease their services and leave without refunding any monies if there are extreme adverse working conditions and the client fails to remedy the situation after it has been brought to their attention. Adverse conditions include but are not limited to destructive, violent, or extreme inappropriate behavior of a child, adult or pet, illegal activities, or other dangerous situations.
Photo Consent: By having your/your child’s braided or face painted and photographed, permission is thereby granted to use images for my portfolio; marketing materials such as print, website; or other publications, contests and competitions, etc. No names are ever revealed. If for any reason you do not agree, please let me know, and your privacy will always be respected.
Guests and Children: Under no circumstances will ANYA Braid Bar supervise children. Their behavior and safety is the client and/or parent's responsibility.
Artwork Limitations: If in public, artist will NOT paint any part of the body that is not legal to expose, or any place she does not feel comfortable painting. Artist will not paint any subject matter she deems offensive, disturbing, or inappropriate for the type of event.
Liability Statement
ANYA Braid Bar stylists and artists are fully vaccinated against COVID. They will choose to wear masks at their discretion and under no circumstance may a client or any attendee at an event insist that a stylist/artist remove their mask. Client is aware that mask wearing is a stylist/artist’s choice.
Artists and stylists only use cosmetic supplies that are current with FDA requirements for use on skin and hair and are not liable for allergic reactions to paint/hair spray. For sanitary reasons, our artist will not paint anyone who is or appears to be sick (cough, runny nose, fever or suffering from cold sores, conjunctivitis, ringworm, sunburn, eczema, any infectious skin condition, or open wounds. Artist uses reasonable care but is not liable for paint damage to clothing or property. Client will pay for any accidents, injuries or damage to equipment or supplies caused by guests, pets, sprinklers, etc.
